Comparative Analysis of Bone Graft Alternatives in Spinal Fusion

Abstract
Purpose: We used hydroxyapatite, allogeneic bone, and demineralized bone matrix mixed with autogenous iliac bone in posterolateral spinal fusion and examined union rates to evaluate efficacy of each material as bone graft extenders.

Materials & Methods: From May 2003 to September 2006, 45 patients had decompression and posterolateral fusion. In 21 cases (19 patients, group I), hydroxyapatite with autogenous iliac bone was grafted in the right side of spine, and frozen allogeneic bone with autogenous iliac bone in the left. Demineralized bone matrix mixed with autogenous iliac bone was grafted in both sides in 26 cases (26 patients, group II). Christiansen classification was used in the determination of bony union.

Results: In group I, complete union was present in 19 cases out of 21 (90.5%) in the right side, and 16 cases out of 21 (76.2%) in the left. In group II, complete union was present in 20 cases out of 26 (76.9%).

Conclusion: Hydroxyapatite, demineralized bone matrix, and allogeneic bone in adjunct to autogenous iliac bone showed union rates of 90.5%, 76.9% and 76.2% each. However, the differences were not statistically significant (p=0.4). We believe that hydroxyapatite and demineralized bone matrix could serve as useful bone graft extenders in posterolateral spinal fusion.
